8. *Previous Visa Refusals (if any):*

* Details of refusals from other countries

9. *Family Details:*

* Father: Name, Date of Birth, Place of Birth, Current Occupation/Status, Current Address
* Mother: Name, Date of Birth, Place of Birth, Current Occupation/Status, Current Address
* Siblings: Names, Dates of Birth, Places of Birth, Occupations/Statuses, Addresses for all brothers and sisters
